A driver's license can be issued by any State or territory of the United States (including the District of Columbia, Puerto Rico, the U.S. Virgin Islands, Guam, the Northern Mariana Islands, American Samoa, and the Commonwealth of Northern Mariana Islands) or by a Canadian government authority, and is acceptable if it contains a photograph or other identifying information such as name, date of birth, gender, height, color of eyes, and address.

  1. Document Category: Identity Document
  2. Document Type: Driver's License
  3. State: Select Issuing State from Drop Down Box
  4. Authority: Typically printed on the back of the license, examples include "Division of Licensing, and Department of Motor Vehicles"
  5. DATES: DO NOT USE MILITARY FORMAT YYYYMMMDD
    1. Issued Date: Can be found on front and back of card depending on State
    2. Expiration Date: Typically on the front of the card
  6. Driver's License Number: Found on the front of the card. Typically in a larger font and in a prominent position near the top.
