From your agencies RAPTOR Credentialing System log in screen, click on the Forgot Username / Password. The system will ask you for information to include your place and date of birth, and address. It will match this information to the information in the Database and if it matches EXACTLY will generate a list of secret questions that you selected during the enrollment process. After you have successfully answered those questions the system will give you your user name and allow you to reset your password. If you are not able to meet these requirements you can contact your Agency Administrator and ask them to reset your username and password.
